CUSR Scholarship Form
The scholarship program is designed to provide families of the CUSR Member Agencies an opportunity to be involved in fun, enjoyable, quality programs that they may not be able to afford without assistance. Applications are completed on Smartrec/Amilia using your Champaign Park District account.
CUSR Annual Information Form (AIF)
The AIF contains extremely important participant information which is necessary for CUSR staff to plan and execute safe and enjoyable programs. CUSR Alcohol Waiver
To ensure the safety of participants Champaign–Urbana Special Recreation (CUSR) requires this form be completed by all adult participants at age 21 and above if they plan to consume alcohol on a trip in an establishment that sells alcohol. This form is to be completed for every trip.
