CUSR Return to In-Person Eligibility
As we move forward in Phase 4 of the Restore Illinois Plan we look forward to offering limited in-person programs. Champaign-Urbana Special Recreation is committed to the health and safety of all employees, guardians, community members, and participants.
In order to best prevent the spread of COVID-19 in our workplace and within agency programs, procedures have been put into place utilizing the most recent information from local and federal health organizations. CUSR Scholarship Form:
The scholarship program is designed to provide families of the CUSR Member Agencies an opportunity to be involved in fun, enjoyable, quality programs that they may not be able to afford without assistance. Click here to download the application form.
CUSR Annual Information Form (AIF):
The AIF contains extremely important participant information which is necessary for CUSR staff to plan and execute safe and enjoyable programs. Each participant needs to have this form completed about them. Click here to get the form that needs to be updated about each participant at the beginning of May each year.
CUSR Registration Form:
CUSR is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. Please click here to download the form to register.
CUSR Medication Dispense Form:
This form must be completed for each program session or when medication changes. Click here to download the Medication Dispensing Form.
CUSR Alcohol Waiver:
To ensure the safety of participants Champaign – Urbana Special Recreation requires this form be completed by all adult participants at age 21 and above if they plan to consume alcohol on a trip in an establishment that sells alcohol. This form is to be completed for every trip.
CUSR Afterschool Parent Handbook 2020:
Manual that applies to all Champaign-Urbana Special Recreation (CUSR) afterschool programs and should enable you to become more familiar with our policies and procedures. Please read through this manual, complete and sign pages 11,12, 14, 15, 16-18, and 20, and return to CUSR. These pages can be completed and signed electronically for your convenience. Please keep the remaining pages for your records.
